Second Membership being created

EXAMPLE OF PROBLEM:

1) User gets expiration email notification 12 days before expiration.
2) User renews membership.
3) There are two memberships now, one expires in a year and another expires in 11 days.

Having two memberships is confusing. Is this user-error or do I need to adjust the configuration in some way?
