The popup on our Unsubscribe-All link appears to be broken. (see attached snapshot).

I've figured out that by adding a stylesheet to the templates main file, some is corrected however the buttons still don't come up so they can press go. (see second screenshot)

It looks like this is likely not the right CSS either since the popups for Unsubscribe-All and normal unsubscribe don't seem to match.
Any thoughts on how to correct?
