Mini Registration Issue

How are you?

I have had a Issue with the Mini Registration form for a while now, in fact for ever since i have started my site at http://cayievents.com/.

It doesn't mater what i do it will not show properly. I go into the back end and ensure that it is checked to show and it does not show properly. The problem i am having is the field to input the information like your name, email etc. does not appear.

I also check to see if there is a file missing using FTP. it doesn't seems to be missing anything.

However, you may see otherwise, so please check, all the FTP and other information needed is provided.

Please help me, i have to be using a third party mini sign up form. that doesn't look as nice as easysocial's.
