keep getting fatal error... pages not rendering properly - since latest upgrade

Issue 1: pages not rendering properly - either just the error shows or a page shows that seems to not include the template

reproduce:
click on blog entry title
click on "subscribers" in komento dashboard

Issue 2:
Just did a test subscription to a blog entry - no notification sent to admin & user was not added to joomla site user list
This just happened... I am guessing this is a configuration problem?
