how i can make a custom field with family link

i want to have, that my profile Type "Private Person" a custom fields to link with Father, Mother, Sister, etc.
look like as multiple text box with dropdown field as family person: to choose: Mother, Father,....... and in addition with a text area to auto complete the User what i type in.
The other user should get a request of this family status.

is that possible? and how to?
