Guest updated their profile picture (!!!!!)

Just seen multiple entries for 'Guest updated their profile picture' on my activity stream. Have added a link in site details below so you can see it.

Any ideas how that could happen!? It seems to have hit the stream multiple times too, but looks like a genuine user account attempt and not spam or anything going by the photo used.
