Can you take a look of my following information and see whether i understand correctly your current issue?

1. UserA submit a new story stream item.
2. UserB add a new comment in this stream item.
3. UserA doesn't get any notification for this.

Another issue is :
1. UserA get notified but when the user click on that notification pop up, it does nothing.
2. UserA will get notified more than once for the same activity.

I've tested 2 test case that my colleague mention above:

Test case 1:
1. As user stackideas I've update one story
2. As another user stackideas2 I've commented on the story http://take.ms/XRybv
3. Expected result was received correctly by user stackideas https://www.screencast.com/t/cGkG5Wy5sX

Test case 2:
1. A user stackideas2 I've commented on the stackideas's story
2. I've click on the onesignal popup message, and it was redirect me to the correct page as shown in this video: http://take.ms/5PVGA
and I've also observed the multiple notification issue with the same activity, seems it doesn't happened.
