Issue is : if you activate website and email fields in settings/comments then those fields will not show in the new comment form for a post.

I added the ftp and site details if you want to check.

I made a fresh install with everything up to date;
(I was mistaken about your demo site, it works fine there)

Don't worry, your answers are fast as light and on week-end even

By default, currently the email and website input field only appear for the guest user.
