I tried creating a new post for approval on your site but I was unable to replicate the issue.
Since the post that you mentioned contains several revisions, that means the post has been published before. The possibility of the user submitting the post for approval again after the post is published is there.

Can you try creating new posts again for approval and see if the issue persists? Let us know if you have found a definite way to replicate this issue.

even if he edited the post a few times, i didnt approve anything before it went live - seems to be a bug as its happened before with the same user's posts

Can you please request your user to submit a test blog post for approval just like how he initially posted his blog post. Once he has done that, please let us know.

I just logged in under his account and submitted a blog post and it doesn't look like it's gone live,However this doesnt answer why the previous ones have gone live automatically...

Unfortunately, we are not able to investigate the issue if we are not able to replicate this. Perhaps you can try asking the user to submit a post approval again along with the steps that he has taken to do so. If the issue persists, you can then update us with more accurate info and steps to reproduce so that we would have a better chance to replicate it.

this has happened again, with another blogger - they submitted a blog post and it went live on the site and is stiil sitting in the queue waiting to be approved. The steps taken are that of any other blog post using easy blog.

I have tried to replicate this again on your site and I failed to do so. The post that I have sent for approval is not published as expected.
If you are able to replicate this yourself, perhaps you can record a video on how to replicate the issue.
