Based on your settings, you have set the system to check on optional fields as well(http://take.ms/318X4). This means it will not be 100% until the user fills up all fields in the profile.

If you want the system to calculate the percentage on 'Mandatory' fields only, you can disable that setting.

there is one place where it asks for information, without a text box for the information. Eg. “Note: Set a description”

This is actually the description for the custom field which you can set.

In your workflows, you can edit the description or disable this setting for those custom fields which you do not want to show the description(http://take.ms/0AcEf).
