But I already allow notifications. The button still off.
I cleared all browser histories & cache. Try it again, allowed notifications, but button still off.
I tried several times both in pc & mobile Chrome browser.

Did I missed something?

From what I check, it seems like it trying to load this file https://munawwarah.net/OneSignalSDKWorker.js but it failed, because the correct file path will be https://munawwarah.net/push/onesignal/OneSignalSDKWorker.js

Thanks, I believe you already corrected the correct path from your Onesignal app, so the above issue that I mentioned in the screenshot, it can request the correct file path now https://munawwarah.net/push/onesignal/OneSignalSDKWorker.js .

But when you manually access it, it shows a 403 Forbidden error, which mean this file can't access in the public area, so I already help you updated these files to 644 chmod permission, so it should work fine now.

Can you give it a check again?

Works well so far on PC and Android.
In iOS, I can add the icon to Home Screen using Mobile Web Template but don't received any push notifications.

Look like iOS device still can't support this push notification yet, you can read the documentation from OneSignal site
