It seems like you haven't setup Facebook app from your test site, am i missing something here?

By the way, if you would like to allow user to select the profile type before register, you have to set registration Type to 'normal process'.

I set it exactly like what you ask me too. But no changes.

I found out that it need at least 1 user within each profile type. If no user in it then it will not display.

So I create dummy user for each profile type and now it works correctly.
