Module Formatting Stripped

I've attached a screen shot showing that when I try to create a new module, I get a white screen. Please fix so I can use modules.
I updated and cleared cache - it appears problem solved. Thanks guys!

It seems like you still using the older version of Easyblog latest blog module, I have help you disabled it, can you try using 5.0 version of that latest blogs module and see how it goes?
