We just need basic form for guest only, no need login/register and other information, i'm attaching sample form which is in demo site. Attaching same as screenshot, please do help us to set up. Thank you.

I am really sorry for the delay of this reply as it is a weekend for us here.

I already help you enabled this setting from backend > Komento > Setting > general > Load Komento's jQuery Library - YES, it should work fine now.

If you not allow new user register on your site, you have to disable this allow registration setting from backend > user > manage > option > allow registration - NO, you can refer on my attached screenshot below.
