I am really sorry for the delay of this reply as it is actually a public day off for us here. I can't seem to bypass the password protection area at http://take.ms/yTVkv to review your custom field settings but I believe the problem is in the custom field settings.

Is it possible for you to provide us with the password for the first screen so that we can check on this for you?

After spending some time investigating this, it seems like the problem is coming from RSFirewall, http://take.ms/vAKrF . Specifically, this option in RSFirewall, http://take.ms/R2wNo

It is converting e-mail addresses even on placeholders to image, which causes problem with the registration form. I think their plugin should only be replacing proper text to image not every single e-mail it finds on the page.

P/S: I have turned off that option and the form works fine now
