We have a site with moderation set for all new posts. No moderation threshold is set.
However a post was created by someone and a moderation email was sent. When the moderator went to approve/deny the post he got an error and it was already posted publicly on the forum. Is there any reason this may have happened?
Using latest version: 4.0.22
Hi there,

One of the possibility this might happen is when the post was already approved by another moderator, which is why the post would be already public.

But if that's not the case, then perhaps you can try reproducing this issue again and let us know the exact steps.
