I've checked your site and your module and it seems like you haven't set up the position and page for the module to be appear on your frontend, http://screencast.com/t/1tjq7bqnc . Module position is provided by your current template itself and you can find the position by adding ?tp=1 at the end of your site url, for example http://yoursite.com/?tp=1 .
