Duplicate items in feed

Hi folks,

Can't explain it but a user just updated his cover photo and it appeared in the stream four times! Any idea why that might happen?

May i know how to reproduce on this duplicate issues? Because I have tried to upload avatar in this user account "guestadmin", it seems like i can't reproduce this duplicate feed in the stream, check my screenshot below.

I know... It seems to happen at random. I can't reproduce it either. I'll keep an eye on iypt and get back in touch when I have an example.

Clicking on the "update with new items from feed" consistently causes a duplication of the post. When the page is refreshed in the browser, the duplicate disappears.

Any ideas?
