Sorry for late reply to this,
The reason why you mentioned that user do not show avatar is because those user commented on your Joomla article, they do not have gravatar account and avatar image. Perhaps you try using an gravatar account comment on your Joomla article post, it will show avatar.

Sorry but I don't quite get you here. In your current Komento settings, it is configured to use avatars from Gravatar.com and in order to show the avatar correctly, the user need to ensure that their profile is updated with a profile picture on http://gravatar.com

Try take a look of your reply at above. May i know do you want show Gravatar photo or just using Komento default avatar?

The reason that there is avatar on the first person that posted the comment is because the user did upload an avatar on Gravatar.com. The second comment probably didn't upload a profile picture on Gravatar.
