I believe there is a conflict in the themes.php from issue #284370 which may be affecting the issue in ticket #284698. It had to be modified to make the special module work.
Basically, I just updated to 1.4.12 and it resolved HOWEVER before I overwrite the themes.php file when I deploy my customizations back to the hosting server I would like to have it corrected ahead of time. Can you confirm on your end that if the modified themes.php (Attached) will affect the Notes application from 1) showing and 2) creating/editing?
