Regarding this issue, I tried to replicate this issue at my local but I can't reproduce this issue. I've also tried at your site by creating a test group and give the permalink 'test-group-one'. After the group created, i then change the permalink to 'test-group-two' and I don't see where is the duplicate avatar / cover. Can you tell me what is the steps to reproduce this issue? If can, maybe you can capture a screenshot and show us the issue?

Just a follow up on this, this continues to happen..
The last Group created (steps that occured)
- Created Group w/ custom permalink (no auto generated)
- Changed permalink name
- Created hidden menu item for Group

There was a known issue when the group is being saved at the back end but this is already resolved on the latest version. Were you editing / modifying it from the back end?

Yes, I create and modify Groups quite often from the back end

I've tried to replicate in my locally based on your instruction at above, it seems work fine.

Can you try update your Easysocial 1.4.8 to 1.4.10 and see that issue is it still persists?
