All replies are "Pending moderation" after update

Last weekend we installed the latest update and to 4.0.13, after completing all replies have the banner "Pending moderation" and I can't seem to find how to get rid of that.

Moderation has never been enabled on this site before, so why now all current and old replies suddenly have this status? When I view the "Under moderation" option in the backend this is also empty.

I can't give site access, but can you tell me what's going wrong here?

Try clear your browser and Joomla cache and see how it goes?

If the issue still persists can you provide us with your Joomla backend and FTP access so we can better have a check?

So someone from our team had an idea what could be wrong after I posted here. It seems this label is CSS shown and hidden and we use LESS, so the problem was solved with a new compilation of LESS.
