Is there any cache that's on the website? It might be best if you turn it off or just put some exemption on that module if possible.

Yes, I have have cache running on my server, but it's on 5 minutes timer, not on a 1+ hour...

I have a little bit lost here, do you mean that it will not updated after 1 hour ago?
If no, can you elaborate more details regarding with your issues?

By default, if your online user already offline, the user avatar will not appear in the "Online user module". So that will show more precise when the users are online or offline.

The Online Friends module says people are online when they are not. It's saying I have friends online when they logoff for more then one hour ago.

Any update on this Arlex?

Actually that was strange, because sometime can reproduce sometime unable to reproduce the issues.

I have double check today and compare with yesterday, I noticed that when you enable the chat system, then the issues will shown out. (By the way, I also noticed when the user offline, that chat system still show that user still show online status)

I noticed when you disabled that chat system, then everything is working fine, I mean that after the user offline, then "Online friends" application will not appear that offline user. Check my video link here : http://screencast.com/t/zp98DlXCGgH

You may have something here, I just tested and indeed it's working as intended when the chat system is offline :\
