When you approve a friend, the system does not check for the limits. The limit rating checks only occurs when you are requesting to be a friend with another user. I have also checked the codes against the request part and it's actually checking on the correct settings. Not too sure how to reproduce this issue on the site. Can you please advise?

I think that is because right now the settings on 'Friends List' is to 100. Before the issue was spotted, 'Friends List' was set to 5, then we saw that we could not have more then 5 friends even though the friends limit was set to 100. So we changed the friends list to 100 and now we can have more friends. I think there must be a connection.

I have been testing this internally and I have no idea how to reproduce this issue but I also have no idea how to reproduce this issue on the site. Can you please revert the settings and provide us with the user access that you were able to reproduce this issue?

Also, I am very puzzled when you said you hit this error when you are approving a friend request because when I checked the codes, there were no "access" checks made when approving a friend request. The access checking only happens when you make a request to be a friend of another user.

Thank you for trying to work it out. I am not sure how it happened but it did (I even have a screen shot). I guess for now everything seems to be fine and if it happens again I will bring it to your attention.
